Act IV – The Pandemonium Fortress

Quests in Act 4
The Fallen Angel (Quest #1)
- Location – Plains of Despair
- Waypoint – N/A
- Objective – Kill Izual
- Rewards – 2 Permanent skill points
The Fallen Angel is the first quest of Act IV. It involves searching for Izual in the Plains of Despair and killing him. After completing Act III and restocking your supplies in town, head outside of the Pandemonium Fortress into the Outer Steppes and search the edge of the map for the stairs to the Plains of Despair.
Once you find the Stairs, enter the zone and search for Izual(Note – This zone has the potential to be filled with lighting shooting souls. Caution is advised while exploring).
Izual has a cold aura enchantment and hits decently hard. Once you kill Izual, open a portal to town and talk to Tyreal(1) for your reward.
Hell’s Forge (Quest #2)
- Location – River of Flame
- Waypoint – River of Flame
- Objective – Bring Mephisto’s Soulstone to the Hellforge and destroy it
- Reward – Random Gems and a Random Rune From the forge
(Note – If you are playing with friends, make sure they leave the game before the Soulstone is broken or they will not be able to collect the rewards themselves as this completes the quest for them as well.)
Hell’s Forge is the second quest in Act IV. It involves searching the River of Flame for the Hell Forge and Destroying Mephisto’s Soulstone.
After turning in quest 1, restock in town and make sure to pick up Mephisto’s soulstone if you have put it in your stash. Once you have restocked and have the Soulstone, return through your town portal to the Plains of Despair. Once arriving in the Plains of Despair, search the outside of the map for the stairs to the City Of The Damned.
Once you find the City of the Damned, Search the zone for the stairs down to the River of Flame and the City of the Damned Waypoint
(Note – The Waypoint and the Stairs are always next to each other).
Once you find the Waypoint and the Stairs, Enter the River of Flame and search the zone for the Hellforge and the waypoint.
The Hellforge is guarded by Hephasto The Armorer.
Hephasto The Armorer needs to be killed to acquire the Hellforge Hammer needed to break the Soulstone.
Once he is dead and the hammer is acquired, bring Mephisto’s Soulstone to the forge and place it.
(Note – As stated above, Make sure that any friends leave the game before you break the Soulstone so they can get the rewards later in their own game)
After placing the stone, Equip the hammer and click the forge a few times to break it. This should release the souls and gems and a random rune will fly out. After completing this quest make sure you search the area for the waypoint to move on to the next quest.
(Note – your friends may rejoin your game at this point)
Terror’s End (Quest #3)
- Location – Chaos Sanctuary
- Waypoint – River of Flame
- Objective – Kill Diablo
- Reward – Completion of Act IV and ability to move on to Act V
(Note – Your map may be slightly different but the locations are the same)
- 1 through 5 – Seals
- GV – Grand Vizier of Chaos
- LS – Lord De Seis
- IS – Infector of Souls
- P – Pentagram and where Diablo will spawn
Terror’s End is the third and final quest of Act IV. It involves entering the Chaos Sanctuary, Opening the five Seals, Killing the seal guardians, and then Killing Diablo.
After you finish quest number 2, use the River of Flame waypoint to return to town and restock on supplies.
Once restocked, make your way back to the River of Flame via the waypoint area. Once back in the River of Flame, look for an angel named Hadriel that is near the waypoint and is guarding the bridge to the Chaos Sanctuary.
Upon finding Hadriel, Proceed past him and make your way through the bridge maze into the Chaos Sanctuary.
Once entering the Chaos Sanctuary, fight your way through the enemies until you reach the giant Pentagram(P) in the middle of the main room. Upon reaching this Pentagram(P), open a town portal for easy return if you die. there will be two seals to the west, one seal to the north, and two seals to the east of this Pentagram(P).
It does not matter which seals you open first. For this guide we are going to start from the West side and work our way around. Start killing the enemies to the west of the Pentagram(P) until you clear the area around both Seals(1)(2).
When you are ready, open the seals and be ready to fight the Grand Vizier of Chaos(GV) and his minions.
Grand Vizier of Chaos(GV) is Immune to Fire and Extra Strong. Once Grand Vizier of Chaos is killed, fight your way to the northern seal(3).
Once you clear to the seal(3), open it and be ready to fight Lord De Seis(LS) and his minions.
Lord De Seis(LS) will spawn back down towards the Pentagram(P). Lord De Seis(LS) has a random aura and his minions will place curses on you so be careful and have adequate potions. After killing Lord De Seis, Fight your way to the eastern seals(4)(5).
After clearing the area around the eastern two seals(4)(5), make sure you have full health and restocked potions from town. Once you are restocked, Open the seals and be ready to fight the Infector of Souls(IS) and his minions.
The Infector of Souls(IS) and his minions are usually super fast and breathe fire. After killing The Infector of Souls and his minions the Sanctuary will turn red, the screen will shake, and Diablo(P) will spawn shortly after in the middle of the Pentagram(P).
Diablo(P) has a red lightning ability that is super dangerous and does a lot of damage so be sure to run around him in circles if he casts it.
His other abilities include a frontal cone fire attack, a full 360 degree fire explosion that goes across the entire room, a targeted slow, and sometimes he can put you in a jail of bones albeit rare. Make sure you put a new town portal down somewhere away from the Pentagram(P) before engaging.
After killing Diablo, Return to town and talk to Tyreal(1). After talking to Tyreal(1), He will open a portal to Horragath which takes you to Act V.
